Registering your child:

Please provide the following documents to enroll your child at Western Valley Elementary:

1. Birth Certificate - To be eligibleto start kindergarten, a student must be five years of age by August 31st of the current school year. The entrance age for a first grader who has not attended kindergarten is six years old prior to August 31st of the current school year. A birth certificate for verification of age is required at the time of enrollment.

2. Immunization Records - Each student attending public school in Arizona must have on file in his/her school verifiable documentation of immunization information. The law requires written documentation which indicates the student's name, date of birth, month, day, and year of all doses received, and the name of the health care provider who administered the vaccine. 3. Proof of Residence - The following documents are Acceptable documents to provide proof of residence:

A. Current utility bill, home contract or lease OR

B. A notarized letter from owner/renter indicating names of people who are living with the owner/renter and the anticipated length of time of residence with owner/renter.

**NOTE: Owner/renter proof must be documented with a current utility bill. (This does NOT include driver’s license, cell phone bill, car registration, or insurance bill.)
